Forte and Tablada performed CE&I services utilizing the federal/DOTD process for road improvements. Tasks for CE&I services included letting results review/comparison, state project filings, AASHTOWare Project SiteManager work, daily site inspections and work reports, change order justification/incorporation into construction design, shop drawing submittal review, quality control, daily contractor & entity coordination/management, daily traffic log diary verification, coordination of required samples and testing, as well as production of 2059 testing binder, and project closeout with DOTD’s final audit. Project closeout binder submission includes signed original schedule of work items, signed original recapitulation of weather and workdays, field book spreadsheets, signed and corrected as-built plans, in and out email correspondence, contractor’s construction layout, signed certificate of release, signed original 2059 form (with mix designs, plant reports, and CDs/CCs/CAs), signed original change orders, clerk filed final acceptance package, and traffic control logbooks.
Forte and Tablada, Inc. was responsible for topographic survey and design for repairs and improvements of 1.375 miles of road located on Thibodeaux Road, from its junction with Planchet Road to its junction with Morgan Road. This roadway serves as a main connection in the loop for LA 37, a roadway that is important to meeting the transportation needs of the local community. The work associated with this project included grading, drainage structures, milling asphalt concrete, lime treatment, in-place stabilized base course, superelevation and cross slope adjustments, asphalt surface treatment, asphalt concrete overlay, permanent pavement markings, raised pavement markers, as well as permanent roadway signage and warning signs.
